Pinoko is a fictional anime character and a major character in the Black Jack series. She is a small girl with pigtails, rosy cheeks, and big, bright eyes. Her most notable feature is her small and doll-like appearance, which comes from being only five inches tall. She wears a pink and white dress with a matching bow on her hair, and usually walks around barefoot. Despite her small size, she's said to have a big personality, and her high-pitched voice usually shows excitement and enthusiasm.

Pinoko's story begins when she was just an unusual growth that Black Jack, the main character, removed from the body of a young girl named Sharaku. The growth was actually a fetus that had been inside Sharaku's twin sister, who died in the womb, and due to Black Jack's expertise, it was able to be removed and survive. However, the fetus was never fully formed, so Pinoko became a five-inch-tall doll-like creature. Black Jack decided to take Pinoko in and raise her as his own daughter since she had nowhere else to go. Despite being only a few inches tall, Pinoko has a big heart and does her best to help Black Jack with his medical expertise.
